About This MicroSim
This MicroSim compares four healthcare payment models — fee-for-service, bundled payment, capitation, and ACO shared savings — to show how each reshapes provider incentives and, through them, cost and quality. Fee-for-service pays per service and rewards volume; bundled payment fixes a price per episode and rewards efficiency; capitation pays a fixed amount per member and most strongly rewards prevention; and ACO shared savings rewards lowering total cost of care while maintaining quality. The bar panels compare all four across cost per patient, health outcomes, preventive services, and ED visit rate.
