Multidisciplinary Care Team Graph Model

About This MicroSim
This graph models the full team caring for one complex patient with diabetes, coronary artery disease, and chronic kidney disease. The patient sits at the center, connected by typed relationships to a primary-care provider who MANAGES_CARE, three specialists who CONSULT_FOR, and four allied-health professionals (care coordinator, pharmacist, dietitian, social worker) who SUPPORT and COORDINATE care; each professional in turn WORKS_AT a facility. The radial structure makes the breadth of a coordinated care team immediately visible.
How to Use
Read each spoke from the central patient: green for the primary-care relationship, orange for specialist consults, and purple for allied-health support, with facilities in gray. Trace how many distinct people and places are involved in one patient's care — exactly the coordination complexity a graph database is built to manage and query. Drag nodes and use the navigation buttons to rearrange the layout.
